Resource and Discharge Planner - Remote

Overview

The Resource and Discharge Planner (RDP) is passionate about working with Young Adults and Adults to deliver compassionate care and provide them with a strong resource network. The RDP will be responsible for meeting with patients to discuss structure and time management, facilitating aftercare planning, and supporting primary therapists with administrative tasks.

In Short

• Actively pursue community programs and events for patients.
• Develop relationships with local resources, schools, and therapists.
• Manage referral lists and ensure accurate information.
• Work closely with management to implement outreach strategies.
• Maintain CRM-based tracking of relationships.
• Teach executive functioning strategies and coping skills.
• Provide vocational support and transition planning.
• Empower patients to create daily and weekly goals.
• Collaborate with the treatment team on patient needs.
• Conduct clinical intakes and/or groups as needed.

Requirements

• Master’s degree in counseling, psychology, social work, or related field.
• Excellent communication and time management skills.
• Ability to problem solve and think creatively.
• Organizational skills and flexibility.
• Knowledge of coping skills and motivational interviewing.
• Ability to manage numerous patients with varying needs.
• Core skill set in CBT and DBT is ideal.
