Overview

This temporary position involves developing and managing care plans for Medicare Dual Eligible Special Needs Plan (D-SNP) members, working within a multidisciplinary team to promote optimal health outcomes.

In Short

  • Experience in social service within healthcare settings preferred.
  • Remote interview process via Microsoft Teams.
  • Hybrid work environment with some in-office presence required.

Requirements

  • Ability to assess psychosocial needs effectively.
  • Strong customer service skills.
  • Advocacy skills for quality care.
  • LCSW license in good standing.
  • Master’s degree in Social Work or related field.
  • Two years of relevant social service experience.
