Senior Resource Specialist - Remote

Overview

The Senior Resource Specialist at Centene will support the Pennsylvania Health & Wellness market by conducting reviews and assessments for the Medicaid Long-Term Services and Supports (LTSS) population, connecting members to local resources, and improving health outcomes in the community.

Requirements

• Bachelor's degree required.
• 4 – 6 years of related experience or equivalent experience.
• Experience in social work, care coordination, or case management preferred.
• Strong communication and interpersonal skills.
• Ability to assess needs and develop care/service plans.
• Experience working with community resources and services.
• Knowledge of Medicaid and LTSS programs.
• Ability to work independently and as part of a team.
• Proficient in using technology for documentation and communication.
