The OB Care Navigator plays a crucial role in transforming the health of individuals by providing personalized care management and coordinating healthcare services for members.

In Short

• Develop and coordinate care management activities based on member needs.
• Assist members in understanding services and benefit options.
• Evaluate member needs and recommend resources for care.
• Provide psychosocial support and access to local resources.

• Document and maintain member information for compliance.
• May perform on-site visits for assessments.

Requirements

• Bachelor’s degree and 2-4 years of related experience required.
• Graduate from an Accredited School of Nursing preferred if holding clinical licensure.
• Equivalent experience in relevant skills is acceptable.
• Current state’s clinical license preferred.
