Maternity Navigator - Remote

Overview

Cayaba Care is looking for a full-time Maternity Navigator (MN) to be a trusted partner and advocate for expectant and new mothers in Memphis. Our MNs are the heart of our work—hired from the communities we serve, they build deep, meaningful relationships with the families they work with.

In Short

• Act as a bridge between the healthcare system and the community.
• Meet with mothers virtually and in-person to educate them on critical health topics.
• Work as part of a multidisciplinary care team.
• Maintain clear and accurate documentation of all care interactions.
• Build trusting relationships with mothers.
• Connect mothers to community resources.
• Bring insights from the community back to Cayaba Care.
• Attend administrative meetings and follow policies and procedures.

Requirements

• 3+ years of experience in community health or patient advocacy.
• Deep understanding of challenges faced by expectant mothers.
• Strong communication and listening skills.
• Problem-solving skills and ability to manage multiple priorities.
• Comfort working independently in the field.
• Valid driver's license and reliable transportation.
• Access to home internet and a phone with internet access.
• Tech-savvy and able to learn technology platforms.
