Overview

The Cadence Health team seeks a remote-based Part Time Nurse Practitioner to serve as the point person to help patients navigate their remote care journey, managing treatment plans and medications in collaboration with the patient’s physician.

Requirements

• 5+ years experience treating patients with chronic diseases.
• Active multi-state compact license.
• ANCC or AANP certification required.
• Master’s Degree as a Nurse Practitioner.
• Experience managing CHF with GDMT.
• Ability to work independently and thrive in a remote environment.
• Passion for patient experience and healthcare innovation.
• Experience with remote patient monitoring technology preferred.
• Experience in a startup environment preferred.
