Overview

The Cadence Health team seeks a remote-based Nurse Practitioner to serve as the point person to help patients navigate their remote care journey, focusing on chronic disease management.

In Short

  • Oversee monitoring of patient vitals and symptoms.
  • Manage treatment plans and medications.
  • Respond to patient escalations identified via RPM.
  • Lead regular virtual check-ins with patients.
  • Support primary care and cardiology offices.
  • Ensure exceptional healthcare interactions.
  • Support internal growth efforts for chronic care delivery.
  • Shape the culture of a growing team.

Requirements

  • 5+ years experience treating chronic diseases.
  • Active Hawaii NP License.
  • Multi-state compact license is a plus.
  • ANCC or ANNP certification required.
  • Master’s Degree as a Nurse Practitioner.
  • Experience managing CHF with GDMT.
  • Stable wifi for remote work.
