Requirements

• Compact multi-state licensure (RN compact).
• 4+ years experience with CHF, hypertension, and diabetes patients.
• Passion for patient experience and healthcare innovation.
• Independent thinker/operator.
• Ability to monitor patient vitals and identify clinical needs.
