Requirements

• Active Practical/Vocational Nursing licensure in NY, MA, CT.
• Experience in primary care preferred.
• Strong technical skills in EMR/EHR systems.
• Excellent organizational and communication skills.
• Ability to work independently and manage multiple tasks.
• Understanding of chronic disease management and patient care barriers.
• Coordination with healthcare providers and specialists.
