Join our team as a Full Time Referral Coordinator, where you will serve as a vital link between clients, providers, and community partners to ensure timely, accurate, and compassionate coordination of services.

In Short

• Professionally manage a high volume of inbound calls from clients, families, and referral sources
• Conduct initial screenings and route calls according to clinical protocols
• Accurately enter referral, demographic, and insurance information into the Electronic Health Record (EHR)
• Coordinate and schedule outpatient intake appointments
• Verify insurance eligibility and benefits when possible
• Communicate appointment details and next steps clearly and respectfully
• Maintain accurate documentation in compliance with HIPAA, licensing, and payer requirements
• Collaborate with internal teams and external partners to ensure seamless care coordination
• Participate in ongoing training and quality improvement initiatives
