Outreach Specialist - Remote

Overview

The Outreach Specialist-CACP is responsible for conducting outbound and inbound calls to engage patients in scheduling Advanced Care Planning (“ACP”) appointments.

In Short

• Conducts outbound and inbound calls to educate patients about Advanced Care Planning.
• Overcome Patient concerns and objections regarding Advanced Care Planning.
• Responds to multimodal communications from Patients.
• Maintains HIPAA compliance and achieves key performance metrics.
• Works flexible shifts within business hours of 8 AM – 8 PM EST.
• Excellent communication skills are essential.
• Proficient computer skills required.
• Must have reliable high-speed internet access.
• Bilingual in English and Spanish is preferred.
• Position is 100% remote.

Requirements

• High School Diploma.
• 2 years of experience in healthcare.
• 1 year of experience in telephonic patient outreach, customer service, or sales.
• Regular and predictable attendance is required.
• Ability to engage with an older demographic compassionately.
• Self-motivated with the ability to work independently.
