Care Manager, RN - Remote

Overview

The Care Manager will work with Aledade primary care practices in Maryland as a part of the Maryland Primary Care Program, providing telephone-based health coaching, quality improvement, and care coordination for Medicare patients.

• Conduct comprehensive assessments of patients' medical, behavioral, pharmaceutical, and social needs.
• Provide education and self-management support tailored to patients' learning styles.
• Coordinate care and act as an advocate for patients and their families.
• Collaborate with primary care physicians and interdisciplinary care teams.
• Provide clinical oversight to non-licensed support staff.
• Support the implementation of Aledade initiatives for population health management.
• Measure and improve quality outcomes for patients.
• Travel to assigned practices and team retreats as required.
• Must be a current Registered Nurse in Maryland.
• 3-5 years of direct healthcare experience preferred.

Requirements

• Current Registered Nurse in Maryland.
• Experience in home health, ambulatory care, or case management.
• Familiarity with the healthcare community or commitment to learn.
• Population health or managed care experience preferred.
• Strong communication skills, both written and oral.
• Ability to work with vulnerable populations.
• Competent in documenting in electronic health records.
• Ability to navigate ambiguity and solve problems.
• Proactive and collaborative work ethic.
• Curiosity and willingness to learn from feedback.
