The Case Management Specialist will support Rare Disease patients and families through comprehensive care coordination and case management expertise, focusing on personalized care plans and addressing barriers to treatment.

Requirements

• Bachelor’s Degree required.
• 3-5 years of patient-facing or customer interaction experience.

• Understanding of health insurance benefits and relevant regulations.
• Experience with Salesforce CRM is a plus.
• Ability to work independently and collaboratively in teams.
• Strong business acumen and strategic thinking abilities.
• Experience in healthcare systems or patient services is preferred.
