Overview

The RN Care Coordinator (Case Manager) at Brigham and Women’s Hospital is responsible for managing patient caseloads and ensuring effective care coordination to achieve desired clinical and financial outcomes.

Requirements

• Bachelor of Science in Nursing required for newly licensed nurses.
• 1+ years of acute care experience required.
• 1 year of acute hospital inpatient case management experience required.
• Strong clinical assessment skills.
• Excellent interpersonal and communication skills.
• Ability to work collaboratively in a team.
• Strong organizational skills and ability to prioritize tasks.
• Bilingual (English/Spanish) preferred.
• Ability to handle a stressful work environment.
