Overview

The Licensed Clinical Social Worker II (LCSW/LICSW) is a key clinical member of the High-Risk, High-Cost Complex Care Management Program, dedicated to preventing unnecessary emergency department visits and hospitalizations through proactive, patient-centered care.

Requirements

• Bachelor’s or Master’s degree in Social Work required.
• 2-3 years of experience in Behavioral Health or Medical Social Work.
• Expertise in trauma-informed care and crisis intervention.
• Familiarity with HEDIS measures and Medicaid/Medicare care models preferred.
• Experience with EHR systems, Epic preferred.
• Strong communication and interpersonal skills.
• Ability to work autonomously and in a team setting.
• Knowledge of social work theories and ethical guidelines.
• Ability to adapt in fast-paced environments.
• Reliable and accountable in patient care.
