Wider Circle is a community based organization that comes to members’ neighborhoods, building trust to engage in places plans and providers can’t reach. We are an extension of the health plan, acting upstream to address Social Determinants of Health, improve outcomes and lower the cost of care.

• Coordinate and refer eligible members to community health workers.
• Collaborate with stakeholders, including informatics team and Chief Medical Officer.
• Develop, audit, and implement tailored care plans.
• Maintain accurate records of member interactions and referrals.
• Prepare reports for program evaluation and billing.
• Ensure compliance with legal and ethical standards.
• Focus on operationalization and billing of CHW services in California.

Requirements

• Licensed Clinical Social Worker (LCSW) certification.
• Experience in community health or social work.
• Strong data assessment skills.
• Ability to coordinate care effectively.
• Understanding of legal and ethical standards in social work.
