Overview

As a Full Time Clinician at Signify Health, you will provide In-Home Health Evaluations, engaging with patients in their own homes and helping bridge gaps in care.

• Provide in-home health evaluations to patients.

Requirements

• Active, unrestricted license in the coverage area.
• Board certification required.
• Multi-state licenses preferred.
• Ability to cover primary locations including Minnesota.
