Overview

As a Per Diem Clinician with Signify Health, you will provide in-home health evaluations, engaging with patients to improve their health outcomes in a comfortable setting.

In Short

• Conduct in-home health evaluations for patients.

Requirements

• Active Nurse Practitioner or Physician Assistant license.
• Board certification is required.
• Multi-state licenses preferred.
• Ability to cover the Greater Seattle area.
• Experience in patient care and health assessments.
• Strong communication and interpersonal skills.
• Commitment to improving health outcomes for patients.
• Flexibility in scheduling and travel.
• Ability to work independently and manage time effectively.
