Licensed Clinical Social Worker (LCSW) – Behavioral Health & Interdisciplinary Care Management - Remote

Overview

We are looking for a Licensed Clinical Social Worker (LCSW) to provide behavioral health services and support social determinants of health as part of an interdisciplinary care team. This role involves conducting remote patient interactions via phone and video, performing assessments, and delivering care management services.

In Short

• Serve on interdisciplinary care team as Behavioral and Social SME.

Requirements

• Bachelor’s or Master’s Degree in social work.
• 3 years of clinical social work experience.
• Valid and current LCSW license.
• Value Based Care experience preferred.
• Excellent listening and communication skills.
• Ability to make assessments and develop care plans.
• Demonstrate a positive attitude and professional customer service.
• Follow HIPAA guidelines and regulations.
• Comfortable with digital technology.
• Ability to think critically and solve problems.
