Licensed Social Worker - Behavioral Health Case Manager - Remote

Overview

The Licensed Social Worker Case Manager will work in a remote and field setting supporting the Medicaid SMI (Severe Mental Illness) Population in South Carolina, participating in care team meetings and ensuring care plans are tailored to members' health needs.

• Work remotely and in the field with Medicaid SMI population.

Requirements

• Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) Program completion or Bachelor's/Master's Degree in relevant fields.
• 1-3 years of experience in case management or related settings.
• Active, unrestricted license required.
• Valid driver’s license and reliable transportation.
• Preferred 3-5 years of experience in case management.
• Preferred licenses include LCSW, LPC, or similar.
