Licensed Clinical Social Worker (LCSW) - Perinatal Mental Health - Remote

Pomelo Care is a multi-disciplinary team of clinicians, engineers and problem solvers who are passionate about improving care for moms and babies. We are transforming outcomes for pregnant people and babies with evidence-based pregnancy and newborn care at scale. Our technology-driven care platform enables us to engage patients early, conduct individualized risk assessments for poor pregnancy outcomes, and deliver coordinated, personalized virtual care throughout pregnancy, NICU stays, and the first postpartum year. We measure ourselves by reductions in preterm births, NICU admissions, c-sections and maternal mortality; we improve outcomes and reduce healthcare spend.

In Short

• Provide direct patient care and care navigation for perinatal mental health issues.
• Support emotional, mental and behavioral care for pregnant people.
• Conduct one-on-one virtual care visits.
• Perform behavioral health screenings and interventions.
• Establish individualized care plans based on evidence-based standards.
• Identify and address patient barriers to care.
• Document all care interactions in a timely manner.
• Collaborate with multidisciplinary teams to meet patient needs.
• Support group care as needed.
• Work full time, Monday to Friday, 8am-5pm.

Requirements

• Independently licensed as an LCSW, LMFT, LPC or LMHC for at least 5 years.
• Completed the NCMHCE, or both NCE & NCMHCE.
• At least 120 hours of clinical supervision completed.
• 5-8 years of experience providing therapy services in healthcare.
• Experience with perinatal mental health care.
• Strong organizational skills and ability to prioritize.
• Facile with email, spreadsheets, and EHRs.
• Understanding of birth inequity and structural racism in maternal health.
• Experience working with Medicaid clients.
• Willingness to cross license in other states.
