Per Diem Physician for Chronic Disease Management - Remote

Overview

VitalCheck Wellness is seeking a Physician to provide chronic disease management programs to a private client in Milford, MA. The Physician will participate in a structured multi-week treatment and coaching program for Chronic Metabolic Diseases, designed to improve their wellness and lifestyle.

In Short

• Provide chronic disease management to patients based on program guidelines.
• Educate patients around healthy eating and lifestyle.
• Prescribe chronic disease management medications.
• Advise/refer patients for follow-up care.
• Must be licensed in Massachusetts; Compact state license is a plus.
• MD or DO degree required.
• 3-5 years of work experience following post-graduate training.
• Background in family practice, internal medicine, or preventive care preferred.
• Proficient with computers and customer service-oriented.
• Personal laptop or tablet required.

• Clean active license to practice in the state of MA.
