We provide our patients with a high-touch, personalized primary care experience, including 24/7 access to your physicians, care coordination services, and more. After launching in 2021, we’ve successfully bootstrapped our first market in Florida with 3 brick and mortar clinics. Now, we’ve launched a joint venture with Tampa General Hospital to proliferate our model across Tampa Bay. In Short

• Seeking a compassionate and highly organized Case Manager.
• Manage Greenbrook’s transition of care program.
• Work closely with physicians to support patients through hospitalizations.
• Lead a program supporting high need patients.
• Oversee Greenbrook’s HEDIS and quality initiatives.
• Establish relationships with healthcare systems and community organizations.
• Work primarily from home with occasional travel to clinics.
• Full-time position, Monday to Friday, 8am-5pm.
• 2+ years of experience as a case manager required.
• LPN or nursing background preferred.

Requirements

• Ability to multitask and work independently.
• Strong knowledge of HIPAA and healthcare regulations.
• Excellent communication and problem-solving skills.
• Strong organizational skills and attention to detail.
• Bilingual in Spanish is a plus.
• Experience with EHRs, including ECW and Epic.
• Familiarity with Google Suite and similar tools.
