Overview

The RN Care Manager plays a vital role in supporting patient care for individuals with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD), coordinating services to optimize care delivery.

Requirements

• Associate’s degree from an accredited school of nursing.
• Current RN license in the state of practice.
• 3+ years of related education, experience, or certification.
• Current BLS or CPR Certification required.
• Efficient and reliable transportation for patient visits.
• Proficiency in electronic medical records and care plan management.
