In Short

• Provide ongoing support to individuals living with dementia and their families.
• Work as part of the Rippl Care Team.
• Provide virtual services including psychosocial assessment and crisis support.
• Support caregivers and patients with psychoeducation and coping skills.
• Manage complex cases and provide consultative support.
• Build strong therapeutic alliances with patients and caregivers.
• Collaborate with an interdisciplinary team for holistic care.
• Drive learning and quality improvement in dementia care delivery.
• Participate in training about neurodegenerative diseases.
• Maintain accurate records of patient assessments and care plans.

Requirements

• Active, unrestricted Texas LCSW license.
• 3+ years of experience in dementia or geriatric care.
• Experience providing virtual services preferred.
• Ability to work collaboratively with an interdisciplinary team.
• Knowledge of evidence-based practices for the geriatric population.
• Strong organizational skills and detail-oriented.
• Innovative problem-solving skills.
• Prior startup experience preferred.
