Overview

The Registered Nurse (RN) in Population Health is responsible for conducting outreach to discharged patients, assessing their recovery, and providing follow-up care to ensure optimal health outcomes.

Requirements

• Associate degree in Nursing; Bachelor's degree preferred.
• One year of experience as a registered nurse.
• Current RN compact/multistate license recognized by the NCSBN.

• Critical thinking and assessment skills.
• Knowledge of telephonic care standards.
• Understanding of patient rights and confidentiality.
