Population Health Registered Nurse - Remote

In Short

• Remote, full-time position at 40 hours/week.
• Core working hours between 11:30am-8pm ET, Monday through Friday.
• 1 weekend day coverage per month required.
• Telephonic outreach to patients for post-discharge assessments.
• Educate and coordinate preventative health screenings.
• Perform chronic disease management and medication adherence education.
• Facilitate coordination of care between health services.
• Accountable to performance targets as an individual contributor.
• Collaborate with Engagement and Population Health leadership.
• Focus on improving patient outcomes.

Requirements

• 2+ years of experience in case management or primary care RN with population health experience.
• Engaging with Medicare and Medicaid populations.
• Nurse Licensure Compact (NLC) required.
• Active RN license(s) in specific states is a plus.
• Ability to leverage motivational interviewing skills.
• Excellent written and verbal communication skills.
• Familiarity with Population Health and HEDIS quality gaps.
• Well versed in Mac iOS and Google Suite.
• Bilingual in Spanish is a bonus!
