In Short

• Lead a team of Care Guides to ensure high-quality, person-oriented care.
• Supervise and mentor the Care Guide Team.
• Create a high-performance team culture and drive achievement of KPIs.
• Manage schedules, caseloads, and performance evaluations.
• Ensure compliance with regulatory requirements.
• Improve care coordination workflows and outcomes.
• Conduct audits to ensure adherence to policies and procedures.
• Provide training on workflows and member engagement strategies.
• Identify opportunities for process improvement.
• Engage in onboarding and ongoing training for Care Guides.

Requirements

• Bachelor’s degree in nursing, Social Work, Case Management, or related field.
• 3-5 years of experience in healthcare management.
• Exceptional communication and collaboration skills.
• Experience with healthcare IT systems and Electronic Health Records.
• Strong critical thinking abilities.
