Overview

The High-Fidelity Wraparound (HFW) Case Manager is responsible for delivering the wraparound process with fidelity, supporting families in managing their services and achieving their goals.

In Short

  • Manage wraparound services for youth and families.
  • Conduct comprehensive assessments and develop service plans.
  • Facilitate team meetings and home visits.
  • Monitor progress and coordinate care plan activities.
  • Engage families in the planning process.
  • Travel 30% within the designated region.
  • Collaborate with multidisciplinary teams.
  • Utilize a strengths-based approach.
  • Provide education and consultation to providers.
  • Ensure data collection for evaluations.

Requirements

  • 1+ years of experience in Total Case Management services.
  • Knowledge of Foster Care and juvenile justice systems.
  • Experience with Behavioral Health Services and Crisis Intervention.
  • Must be a Certified HFW facilitator or complete training within 12 months.
  • Bachelor's Degree in Human Services or related field.
  • Strong relationship-building skills with families.
  • Ability to analyze complex information.
  • Commitment to empowering families.
  • Must reside in specified counties in Kentucky.
  • Willingness to travel 30% of the time.
