Overview

The Case Manager, Registered Nurse in the Healthy Aging Support Program at CVS Health is responsible for telephonically assessing, planning, and coaching Medicare members to help them manage their health effectively.

In Short

  • Assess and evaluate the health status of Medicare members.
  • Provide one-on-one coaching and education on health management.

Requirements

  • Active and unrestricted compact Registered Nurse licensure.
  • 5 years of clinical practice experience required.
  • Preferred experience in Managed Care/Medicare.
  • Bilingual candidates preferred.
  • Strong communication and organizational skills.
  • Ability to work independently and navigate multiple systems.
  • Bachelor's degree in Nursing or equivalent required.
