Overview

The Bilingual Spanish Community Health Worker II will support health equity and improve health outcomes for primary care patients by addressing health-related social needs and care coordination. This role involves collaborating with health teams to provide patient engagement and health coaching, primarily focusing on children and adolescents with obesity and asthma.

In Short

  • Provide health coaching and care coordination to patients and families.
  • Engage patients through telephonic communication, home visits, and office visits.
  • Facilitate group visit sessions based on a set curriculum.
  • Help patients navigate health systems and address barriers to care.
  • Document patient encounters and track progress towards health goals.
  • Work collaboratively with a multi-disciplinary care team.
  • Participate in quality improvement activities.
  • Provide culturally sensitive services to diverse populations.
  • Assist with telehealth technical support and visit management tasks.
  • Maintain communication with healthcare providers regarding patient care.

Requirements

  • High school diploma or GED required; bachelor’s degree preferred.
  • Three years of experience in community health or related fields preferred.
  • Experience in pediatric health management and motivational interviewing is desired.
  • Proficiency in Microsoft Applications is required.
  • Ability to work independently and as part of a team in multicultural settings.
  • Spanish language proficiency is preferred.
  • Strong organizational and time management skills.
  • Ability to engage with diverse communities.
  • Willingness to learn basic clinical concepts.
  • Effective communication and advocacy skills.
