Overview

We are seeking a dedicated and compassionate Community Health Worker (CHW) to join our team at the Institute of Community Health Innovation. This role is essential for managing relationships with patients, local community-based organizations, and community members while providing education, resources, and health information.

Requirements

• High School diploma/GED required.
• Experience as a CHW varies by level (2-9 years).
• Certification from ARCHWA required for Level III.
• Strong interpersonal and communication skills.
• Knowledge of public health principles and practices.
• Skilled in community outreach and engagement.
