Overview

We are seeking a knowledgeable and experienced Michigan Choice Adult Waivers Specialist to join our team remotely. The ideal candidate will have a strong background as an adult care worker and a thorough understanding of Michigan’s Home and Community-Based Services (HCBS) waiver programs.

In Short

  • Assist individuals in understanding and accessing the Michigan Choice Waiver Program.
  • Conduct eligibility assessments, care planning, and ongoing case management.
  • Work collaboratively with healthcare providers and community organizations.
  • Maintain compliance with Medicaid waiver program policies.
  • Document all assessments and service plans according to guidelines.
  • Advocate for participants to ensure necessary support.
  • Provide guidance on Medicaid waiver program requirements.
  • Monitor and review participants' care plans.
  • Stay updated with changes in waiver programs and regulations.

Requirements

  • Minimum of two years of experience in adult care or social services.
  • Extensive knowledge of the Michigan Choice Waiver Program.
  • Experience with older adults or individuals with disabilities.
  • Strong understanding of MDHHS regulations and Medicaid eligibility.
  • Excellent communication and organizational skills.
  • Ability to work independently in a remote environment.
  • Proficiency in electronic health records and Microsoft Office.
  • Bachelor’s degree in social work or related field preferred.
