Overview

The Community Health Connector improves the health outcomes of members with complex healthcare needs by providing support and guidance to access necessary healthcare and social resources. This fully remote role requires bilingual fluency in Spanish and Bengali and involves making a meaningful impact on diverse, underserved populations.

Requirements

• High school diploma, GED, Associate’s or Bachelor’s degree.
• At least 1 year of experience with underserved populations.
• Proficiency with technology for remote communication.
• Fluency in Spanish and Bengali.
• Strong communication and organizational skills.
