Community Supports Coordinator - Remote

Overview

The Community Supports Coordinator will build awareness about the NYHER program and Medically Tailored Meals throughout New York communities, ensuring beneficiaries have access to essential services that can improve their health.

In Short

• Engage with community health organizations to promote MM services.
• Create and maintain a contact list of target organizations.
• Participate in community events and webinars.
• Drive an increase in MTM referrals.
• Work with cross-functional teams.
• Travel regionally up to 50% of the time.
• Utilize Microsoft Office for presentations and reports.
• Stay updated on health department requirements.
• Ensure all contacts are up to date in internal systems.
• Attend county round tables and webinars.

Requirements

• 2 years of community health work experience.
• Ability to engage community health organizations.
• Excellent oral and written communication skills.
• Experience with Microsoft Office suite.
• Willingness to travel regionally.
• Associates or Bachelor’s degree preferred.
• Experience with NY Medicaid is a plus.
• Background in social services or nutrition is advantageous.
