Case Manager - Behavorial Health - Remote

Overview

The Case Manager will support the SMI Medicaid Population in Virginia, working primarily in a remote setting while also conducting home visits for assessments and care coordination.

• Work with members, providers, and multidisciplinary teams.
• Conduct clinical assessments and develop care plans.
• Perform telephonic and face-to-face visits.
• Monitor care plans and document interventions.
• Promote integration of services for members.
• Facilitate interdisciplinary care team meetings.
• Use motivational interviewing techniques.
• Travel locally for member visits.
• Maintain a regular outreach and management schedule.
• Must be licensed in Virginia.

Requirements

• Completion of an accredited LVN or LPN program or a relevant degree.
• 1-3 years of experience in case management or related fields.
• Active and unrestricted license required.
• Valid driver’s license and reliable transportation.
• Preferred experience of 3-5 years in similar settings.
• Preferred licenses include LCSW, CCM, LPC, etc.
