An exciting job role has recently become available at Sefton. They are looking for a new member for their Hospital Discharge Team.

In Short

• Assess patients' needs for safe hospital discharge.
• Develop and implement care plans in collaboration with healthcare professionals.
• Coordinate community services to support patients' transition home.
• Advocate for patients' rights and preferences.
• Regularly review and adjust care plans to meet evolving needs.
• Maintain accurate records of assessments and interventions.
• Collaborate with multidisciplinary teams to provide holistic care.
• Strong communication and empathy skills required.
• Ability to work under pressure.

Requirements

• Social Work England registration.
• Eligibility to work in the UK.
• Full UK driving licence.
• Post-qualified experience.
• Degree or equivalent qualification in Social Work.
