Qualified Social Worker – CHC/D2A Assessments Team - Remote

Overview

Liquid Personnel is currently recruiting for an experienced and qualified Social Worker to join a busy CHC/D2A Assessments Team based in Lancashire.

In Short

• You will gain experience within adult social care and complete mental capacity assessments.
• You will set up packages of care and support planning.
• Identify areas of need and/or risk of harm.
• Ensure appropriate safeguarding practices and measures are observed.
• CHC and D2A based work.
• Stable long-term contract.
• Mostly remote role with only face-to-face visits required.

Requirements

• Experience within adult social care.
• Completing mental capacity assessments.
• Adept at setting up packages of care and support planning.
• Social Work England registration.
• Eligible to work in the UK.
• Full UK driving licence.
• Post-qualified experience.
• Degree or equivalent qualification in Social Work.
