An exciting job role has recently become available in Hillingdon. We are looking for a new member for the Adults MASH Team.

In Short

• Work within the context of the Care Act 2014.
• Provide person-centred Social Work intervention.
• Enable service users and carers to maintain their independence.
• Produce reablement plans for clients.
• Carry out joint working with therapists.
• Knowledge of assistive technology is required.
• Flexibility to work primarily from home.
• Short-term work with no long-term case management.

Requirements

• Social Work England registration.
• Eligibility to work in the UK.
• Full UK driving licence.
• Post-qualified experience in Social Work.
• A degree or equivalent qualification in Social Work.
