An exciting job role has recently become available at the Stockport Metropolitan Borough Council. They are looking for a new member for their Adult/Hospital Discharge Team.

• Collaborate with Pennine Care colleagues to enhance mental health services.
• Ensure safe and timely discharges for individuals with mental health conditions.
• Co-produce Care Act assessments and support plans.
• Conduct Mental Capacity Assessments.
• Chair Best Interests Meetings.
• Contribute to improving the discharge process.
• Remote/hybrid working available.

• Excellent rate of pay up to £30/hour.

Requirements

• Social Work England registration.
• Eligibility to work in the UK.
• Full UK driving licence.
• Post-qualified experience.
• Degree or equivalent qualification in Social Work.
