In Short

• Provide a purposeful, professional social work service to clients.
• Conduct comprehensive assessments of complex health and social care needs.
• Perform risk assessments and management.
• Deliver care and interventions in line with statutory requirements.
• Act as a Care Coordinator following the Care Programme Approach (CPA).
• Collaborate and communicate effectively with other professionals and organizations.
• Ensure community resources and support are available to service users.
• Offer full carers assessments and care plans where appropriate.
• Work within a supportive team environment.

Requirements

• Social Work England registration.
• Eligibility to work in the UK.
• Full UK driving licence.
• Post-qualified experience in social work.
• Degree level or equivalent qualification in Social Work.
