Overview

The Support Coordinator works closely with the participant and his/her family to link them to appropriate supports and services in the surrounding community. An individualized service plan and person-centered planning tool is created for the participant, and the plan is monitored and adjusted by the Support Coordinator as needed. The Support Coordinator is also responsible for assisting the consumer in managing the budget that is allocated to them for various services and supports.

  • Use of Person-Centered planning to assist individuals and their families.
  • Facilitate development and implementation of the Individualized Service Plan.
  • Identify service providers and community resources.
  • Coordinate and develop resources to meet individual needs.
  • Maintain effective communication with individuals and team members.
  • Monitor the individualized Service Plan and budget.
  • Ensure individuals are free from abuse and neglect.
  • Conduct monthly contacts and annual home visits.
  • Report any suspected violations of contract or regulations.
  • Attend regular staff meetings and maintain HIPAA standards.

Requirements

  • Bachelor’s Degree.
  • At least 1-year experience with individuals with disabilities.
  • Proficiency with Microsoft Office and ability to learn iRecord.
  • Valid driver’s license and ability to travel within specified counties.
  • Commitment to Person-Centered Planning principles.
