In Short

• Collaborate with interdisciplinary team members to deliver comprehensive virtual care services to patients.
• Provide support for patients and families coping with life transitions and chronic illnesses.
• Empower patients and caregivers by providing ongoing education regarding psychosocial issues related to kidney disease.
• Conduct virtual assessments to identify patients’ social, emotional, and financial needs.
• Develop and implement individualized care plans connecting patients with community resources.
• Manage referrals for patients at high risk of poor health outcomes.
• Provide supportive counseling services as permitted within the scope of clinical training.
• Report unreported medical or social conditions to the clinical team.
• Advocate for patients by facilitating access to appropriate social services.
• Maintain thorough documentation of all patient interactions and care plans.

Requirements

• Master's degree in Social Work from an accredited program.
• Must hold an active unrestricted Social Worker license in the state of residence.
• Willingness to obtain and maintain additional state licensures as needed.
• 1+ years of experience in social work, preferably in a healthcare setting.
• 2+ years’ experience in care delivery to patients with chronic diseases.
• Knowledge of healthcare regulations and commitment to patient confidentiality.
• Proficient in using virtual care platforms.
• Strong case management skills in a remote care environment.
• Understanding of social determinants of health.
• Excellent communication and interpersonal skills.
