Overview

We’re looking for a Pediatric Behavioral Health Nurse Care Manager to join our Care and Case Management team—someone passionate about supporting children, adolescents, and their families holistically through their healthcare journey with deep focus around behavioral health.

In Short

• Deliver coordinated, family-centered virtual Care Management by phone.
• Develop personalized, goal-oriented care plans in collaboration with families.
• Support members through behavioral health treatment plans and transitions of care.
• Help families navigate behavioral health systems and community-based resources.
• Partner with local pediatricians and specialists for coordinated care.
• Provide compassionate, longitudinal support to families.
• Coordinate resources addressing clinical and social determinants of health.

Requirements

• Bachelor of Science in Nursing (BSN) required.
• Active Compact RN license in good standing.
• 5+ years Nursing experience.
• 2+ years experience in Behavioral Health Pediatric setting.
• Willingness to become licensed in multiple states.
• Comfortable managing pediatric behavioral health conditions.
• Excellent verbal and written communication skills.
• Technologically competent across multiple electronic systems.
• Deep understanding of cultural and socioeconomic factors in pediatric behavioral health.
