Overview

We are seeking passionate Nurse Care Managers to join our Care and Case Management team, focusing on holistic care for members with behavioral health and substance abuse needs.

In Short

• Deliver coordinated, patient-centered virtual Care Management.
• Create impactful care plans with members and the care team.
• Help members navigate complex medical conditions and benefits.
• Partner with local providers for coordinated care.
• Provide compassionate follow-up care and build supportive relationships.
• Assist during acute healthcare episodes with coordinated Case Management.
• Coordinate resources to address clinical and social problems.

Requirements

• Bachelor of Science in Nursing (BSN).
• Current CCM required.
• 5+ years of nursing experience.
• 2+ years in care, case, and disease management.
• 2+ years in Behavioral Health or substance use field.
• Active Compact RN license in good standing.
• Experience working remotely and using medical record systems.
• Empathy and strong communication skills.
• Spanish speaking desirable.
