Registered Nurse Triage and Transition Ambulatory Care PRN - Remote

Overview

This position functions as a vital member of the healthcare team who contributes to the success of the practice by demonstrating customer/patient focus, effective communication, professionalism, and teamwork. It involves assessing, planning, implementing, and evaluating comprehensive care for selected patient populations and their families.

• Perform clinical triage and comprehensive assessments.
• Manage transitions of care to ensure appropriate follow-up.
• Contact patients post-emergency visits to ensure follow-up.
• Educate patients and families on health management.

Requirements

• ASN / ADN from an accredited program.
• Current RN license in Colorado or Montana.
• Current BLS certification endorsed by the American Heart Association.
• Preferred: Bachelor's Degree in Nursing (BSN).
• Preferred: One year of experience in a clinic or inpatient setting.
• Ability to work nights and/or weekends as required.
• Regular attendance during scheduled hours is required.
• Ability to perform essential functions with reasonable accommodations.
