Health Coach - CMA or LPN - Primary Care - Remote

Overview

The Health Coach will assist with population health management, coordinate care, and provide self-management support to patients, while demonstrating leadership and effective communication within the healthcare team.

In Short

• Participates in population health outreach and identifies patients with gaps in care.
• Ensures pre-visit planning and prepares for patient visits.
• Assists providers in addressing care gaps and activating standing orders.
• Documents clinical encounters and assists with computerized provider order entry.
• Uses motivational interviewing to coach patients on self-management.
• Conducts quality data reviews and facilitates outreach to close care gaps.
• Collaborates with care team nurses for complex care management referrals.
• Maintains cleanliness and inventory of medical supplies.
• Performs in-house testing as per established standards.
• Follows compliance with HIPAA and company policies.

Requirements

• High school diploma or equivalent.
• Medical Assistant Certification or Licensed LPN required.
• Completion of an accredited course in medical assisting or two years of relevant experience.
• CPR certification.
• Act 34, 33, and 73 Criminal Background Clearance Certificates.
