Overview

The Health Coach (CMA/LPN) will assist with population health management through patient outreach, coordination of care, and support to patients while demonstrating effective communication and leadership skills.

In Short

  • Participate in population health outreach and identify patients with care gaps.
  • Assist providers during clinical visits and manage patient data.
  • Use motivational interviewing to support patient self-management.
  • Conduct registry and quality data reviews.
  • Coordinate care for high-risk patients with the care team.
  • Ensure compliance with established protocols and standards.
  • Maintain inventory of medical supplies and equipment.
  • Perform in-house testing and other clinical duties as assigned.
  • Mentor team members in office functions and patient care.
  • Adhere to HIPAA and data security guidelines.

Requirements

  • High school diploma or equivalent.
  • Medical Assistant Certification or Licensed LPN.
  • Completion of an accredited medical assisting course or relevant experience.
  • CPR certification.
  • Background clearance certifications (Act 34, Act 33, Act 73).
  • Ability to communicate effectively and build relationships.
  • Experience in patient care and clinical support.
  • Knowledge of population health management principles.
  • Strong organizational skills.
  • Commitment to ethical and legal standards.
