Care Coordination RN - Remote

Overview

Evry Health is seeking a tech-savvy Nurse to join our team for Care Coordination. As a Care Coordinator, you work with members to improve their wellness and engage with our health plan's benefits. This role allows you to build good relationships with both health plan members and medical providers through various communication channels in a fully remote environment.

In Short

• Communicate and provide education on insurance plan benefits.
• Use negotiation and motivational interviewing techniques to increase member engagement.
• Provide pro-active support for members via phone, email, and text.
• Employ active listening skills to handle difficult calls professionally.
• Track and document work using internal software tools.
• Consult with supervisors and medical teams as needed.
• Manage escalations within the department effectively.
• Assist with coding, medical records, and claim processes.
• Interact with external providers to gather clinical information.
• Must reside in the CST or EST time zones in the USA.

Requirements

• 1-2 years of experience with a health plan.
• 3-5 years of nursing experience in a clinical setting.
• Current Texas nursing license or Compact License required.
• Diploma from an accredited nursing school required.
• Knowledge of medical and insurance terminology.
• Experience in cardiology, women's health, or primary care preferred.
• Experience in telephonic outreach and education.
• Innovative spirit with a passion for healthcare change.
• Bonus: Familiarity with Salesforce or similar platforms.
• Bonus: Spanish fluency is a plus.
