Remote Bilingual RN Case Manager (California RN License Required)

We are seeking a remote bilingual Spanish RN Case Manager to join our case management team, responsible for health care management and coordination for members with complex and chronic care needs.

In Short

• Serve as a resource for members, families, and physicians.
• Ensure access to appropriate care and facilitate referrals.
• Complete comprehensive assessments of members' health status.
• Collaborate with care teams to implement care plans.
• Provide education and self-management support to members.
• Assist in problem-solving with providers and service issues.
• Maintain confidentiality of all PHI in compliance with laws.
• Work with providers to assure effective care coordination.
• Requires California RN license and experience in case management.

Requirements

• Minimum 2 years' clinical experience required.
• Minimum 1 year case management experience required.
• Health Plan experience preferred.
• Active RN license in California required.
• Willingness to obtain additional state licenses.
• High understanding of community resources and treatment options.
• Effective written and oral communication skills.
• Ability to work in a fast-paced environment.
• Strong problem-solving and organizational skills.
• Proficiency in PC-based software programs.
